Transfer Center

Please follow the below steps to transfer your patient to Mary Washington Hospital or Stafford Hospital.

1. Call the MWHC Transfer Center at 540.741.6980.

2. Provide us with the following information:

• Demographic information including patient name, date of birth and insurance
• Referring facility name and phone number
• Referring provider name and phone number
• Diagnosis and current medical status
• Requested treatment

3. Send the following documents with the patient:

• Face sheet
• History and Physical
• Imaging and labs
• Discharge Summary
