Continuous Care

We are always striving to find new ways to support you in managing your health and wellness. As part of our commitment to providing the best care possible, a select number of our Mary Washington Primary Care offices are offering a new service. This service helps you manage your health, coordinate care, and ensure all recommended services are provided.

We are partnering with Continuous Care, a trusted leader in virtual care management, to bring you this valuable service. The goal is to streamline your healthcare experience, make sure you receive timely follow-up care, and assist you in managing chronic conditions or ongoing health concerns.

Continuous Care services include:

  • Personalized Telehealth Appointments — Nurse professionals engage in regular telehealth appointments with patients, delivering individualized care and health advice tailored to your unique needs. Connecting virtually via your computer, tablet or smartphone makes care accessible and convenient anytime, anywhere.
  • Patient Advocacy — Continuous Care nurses help facilitate access to resources and specialists, assist with the logistics of appointment scheduling and prescription management. In addition, they work with patients and their families to ensure they understand their diagnoses and treatment options.
  • Remote Monitoring — Continuous Care nurses facilitate remote monitoring of relevant vital signs — like blood pressure, glucose levels, heart rate and oxygen levels — all critical for early detection of potential issues between physician appointments.
  • Coordinated Care and Seamless Communication — The nursing professionals are an extension of your care team. They will keep us informed of your condition(s), progress, and escalate clinical concerns to your physician when necessary.

These services are provided with the full endorsement of the providers and as a complement to your usual care. We do not intend for this program in any way to substitute for the primary care provider relationship you have established. The goal is to provide an extension to the care we can provide so that your needs are more fully and timely met.

Fees for these services are typically covered by Medicare or commercial insurance.
